Bad Checks - Merchant Information Form

Passing a bad check is a crime. If you or your business have been the victim of a person passing a bad check, you have the option to bring the returned check(s) to the Platte County Prosecutor's Office for collection and prosecution. The Prosecutor's Office will attempt to collect on the bad check by sending letters to the person who wrote the check, including a court date notice if they fail to respond in a timely manner. 

The Prosecutor's Office requires proper identification to be able to locate the offender. The following identifiers are required when you accept a check:

  1. Name and current address
  2. Valid drivers' license number
  3. Date of birth

If this is your first time bringing a bad check to the Prosecutor's Office for collection, please fill out the form below and click submit, then bring or mail your checks to:

Once you have submitted your check(s) to the Prosecutor's Office, DO NOT accept payment from the individual. Please advise them that they need to bring or mail their payment (including any additional fees) in the form of a money order to the Platte County Prosecutor's Office to avoid further prosecution.
